Report a new incident
From any location page or the Incidents section, tap 'Report new incident'. The form opens with your current location pre-selected. You can also report directly from the mobile app.
Select category and severity
Choose the incident category — injury, property damage, near-miss, food safety, or other. Then set the severity level: Low, Medium, or High. High-severity incidents trigger instant manager alerts.
Add description and photos
Write a detailed description of what happened, when, and who was involved. Attach photos from your camera or gallery as evidence. The more detail you provide, the faster the resolution.
Manager reviews and resolves
Managers receive an alert for new incidents. They can review the details, add internal notes, assign follow-up actions, and mark the incident as resolved once corrective action has been taken.
Track incident history
All incidents are stored with a full audit trail — who reported it, when, what category and severity, resolution notes, and timestamps. Export reports for WHS compliance or insurance purposes.
